Individual player signing form
All players must be signed before they play. No exceptions - and in the case of under-18's, signatures must be from parents or official guardians. Anything else will be considered as an unsigned player.
From this season all teams are to print their own sheets. They are as in the previous season so all information as before. The only difference is that the teams have to be filled in at the head of the sheet and also the fixture and date.
The date and the teams need to be filled in and then the marks etc for each team.
